Change exclusion commands.

e => set exclusion (radius 1), change radius to LOS, remove exclusion

(Because of the LOS restriction, the intermediary step of 4 is not needed any more.)

E => cycle through exclusions Ctrl-E => clear all exclusions

I also reordered the commands on the 'X' help screen.
